Failure is indeed inevitable. But some calculative measures can avoid a few of them, but not all. Because we can't foresee everything. We learn more from failures than success. But that doesn't mean we fail always. I believe in the power of manifestation. Do not think about failure, but know how to handle them.

So what is paramount here is to convert those instances of failure into wisdom and implement it in our lives. This way, there is no excuse for not acting on it and perhaps give ourselves a chance to let go off guilt
Depends on the situation at that time, Some, due to failure and lack of guidance can feel sad and misguided. And about Accountability depends on person to person will power. i.e how he/ manages thier emotions, mind and all that.
